Home Care

In addition to Respite Care, ACHIEVA provides the following Home Care services:

Home and Community Habilitation
Supports are provided to individuals who need assistance in learning, maintaining or improving their skills in life to live more independently, or to be more productive and participatory in community life.  Services are provided in a manner that ensures an individual’s health and welfare.  In the community, our supports are primarily provided on an hourly basis to individuals who would like to increase or enhance their social, community and/or recreational skills.  Our providers are able to pick up a family member at home, take them out to a restaurant, movie, dance or concert and then bring them home.  Self advocates are encouraged to take advantage of this service to enhance the enjoyment of community life.

Homemaker/Chore
This service enables the individual, or the family with whom the individual resides, to maintain his or her residence when no household member, landlord or agency staff is able to perform routine cleaning, laundry, meal preparation and/or outside yard maintenance.

Transportation and Classroom Aides
Home Care provides transportation for individuals with a disability to get to and from work or day programs when other transportation arrangements are not available.  A bus aide can also be provided to support a school student or Access rider when an aide’s support is temporarily needed due to behavior or medical concerns. ACHIEVA’s Home Care also provides classroom aides to school district’s to assist with life skills, learning and emotional support when a paraprofessional is on leave or when extra support is needed.

Personal Care Services
ACHIEVA can assist a family in obtaining personal care services. These services can provide help with bathing, grooming, getting dressed, toileting, and feeding oneself.  For example, preparing a youngster for the day could take three hours or more. The family may need assistance with other children, or help in being able to get to work on time.
