Community Inclusion Professional

The Community Inclusion Professional is responsible for integrating individuals with disabilities into the community to participate in work recreational, leisure, and volunteer activities. The Community Inclusion Professional will provide training, support needs, and direct care, to the individuals in the community. The Community Inclusion Professional will work with individuals with disabilities to help them achieve their goals based on their individual preferences and abilities.

RESPONSIBILITIES:

  • Understand and implement/follow current ISPs of each person being supported and develop strategies that lead to the achievement of individuals’ personal outcomes.

  • Provide support in a manner which respects a person’s right to privacy and upholds their dignity.

  • Establish good working relationships with family/other team members.

  • Know goals and outcomes and work with the person to achieve their highest potential and goals for community integration and contribution, relationship building, vocational exploration, self-determination, learning and personal growth.

  • Assist people to develop and expand participation in community activities and to build relationships with community members, businesses, and organizations.

  • Plan, prepare and implement recreational, work, and volunteer activities. Develop community building strategies and opportunities through networking with friends, family, neighbors, associates and community members. Organize and participate in individual planning processes to define a community building focus for people supported and ensure the plan is meaningful and beneficial to the person.

  • Demonstrate a person-centered practice and support each person’s choice in the activities, locations, and people with whom that person interacts on a daily basis.

  • Provide transportation to different community locations in a safe manner.

  • Maintain a healthy balance between supporting people’s health and safety and their self-determination. Support people with their personal care needs and activities of daily living as needed.

  • Attend team meetings as a way to facilitate communication and to help sustain a vision of community for each person supported.

  • Participate in training, staff meetings, and supervision meetings as scheduled.

  • Complete and maintain required records and documentation, such as progress notes, attendance, and work logs.

  • Maintains vehicles in a safe, clean and fully equipped condition.

Qualifications

High school diploma or equivalent and experience or knowledge of supporting people with intellectual and developmental disabilities preferred. Effective communication, organizational, interpersonal, and planning skills required. Satisfactory Act 33, Act 34, and FBI clearances required. Valid PA Driver’s License preferred.
